HSAs and Direct Primary Care
Federal changes that took effect January 1, 2026 expanded how Health Savings Accounts can work with qualifying direct primary care memberships. Here are answers to common questions for patients considering Good News DPC in Long Beach.
-
The One Big Beautiful Bill Act changed federal HSA rules for qualifying direct primary care arrangements and certain Bronze and Catastrophic health plans.
These changes can affect both your ability to contribute to an HSA and how you spend existing HSA funds.
-
Beginning January 1, 2026, fees for a qualifying direct primary care arrangement may be paid or reimbursed tax-free from an HSA.
The arrangement must meet IRS requirements. Ask your HSA administrator or tax advisor how the rules apply to your membership.
Please refer to IRS Notice 2026-05 for more information.
-
For 2026, qualifying DPC arrangements can preserve an otherwise eligible person’s ability to contribute to an HSA when aggregate monthly DPC fees stay within $150 for one person, or $300 for an arrangement covering more than one person.
These thresholds concern contribution eligibility. A qualifying DPC fee above the threshold may still be reimbursable from existing HSA funds, even though the membership affects eligibility to make new contributions.
The thresholds are adjusted for inflation after 2026.
Please refer to IRS Notice 2026-05 for more information.
-
Qualification depends on the arrangement’s terms, including the services provided and how fees are charged.
A membership price within the monthly threshold does not, by itself, establish eligibility. Review the arrangement with your HSA administrator or tax advisor.
-
Starting in 2026, qualifying individual-market Bronze and Catastrophic plans available through a health insurance marketplace are treated as HSA-compatible. Certain qualifying plans purchased outside the marketplace also receive this treatment.
You must still meet the other requirements for making HSA contributions.
-
For an individual who pays their own DPC membership fees while enrolled in an HSA-qualified high-deductible health plan, those fees do not count toward the plan’s deductible or out-of-pocket maximum.
-
We can provide documentation of membership payments. Keep your receipts and ask your HSA administrator what records you should retain.
-
The HSA changes do not automatically enroll you in insurance or create a special enrollment period.
Compare plans during the enrollment period available to you, considering premiums, deductibles, prescriptions, provider networks, and your DPC membership fee.
